Little Hearts Home Day Care provides a safe and nurturing environment where infants, Toddlers, and kids can feel ‘Home away from Home”. I appreciate you trusting me with your child’s care. I am committed to making this transition away from home easy and smooth for you and your child.


In addition to caring for your child, everything I do is designed for learning.

Regular Hours of Operation:

Monday to Friday - 7:00am to 5:00pm (or as discussed & agreed between the provider and parent).

Weekend Hours - Please discuss if needed.


Authorized persons will be allowed to pick the child from the center. Proof of ID may be required at the time of checking out.


(A copy of state-issued ID is required of all authorized persons)


Arrival and Departure Procedure:

To ensure the privacy and comfort of all children and families enrolled in my childcare program, I kindly ask that all drop-offs and pick-ups take place at the front entrance door. 


Parents and visitors should not enter other areas of the home unless invited to do so by the provider. This policy helps maintain a safe, respectful, and private environment for everyone in care. Thank you for your understanding and cooperation.


Individual Needs and Group Care 

Every child develops at their own pace and has unique needs. I strive to provide a positive environment that supports each child's growth and learning. However, because this is a group childcare setting, children must be able to participate safely in daily routines and activities with the level of supervision available. If a child's needs require care beyond what can reasonably be provided in the program, I will work with parents to discuss concerns and determine whether the placement continues to be a good fit for the child and the program.


Change of Clothing:

Parents are responsible for providing and maintaining a spare set of clothing for their children. Kids grow fast, please check frequently to make sure the spare set of clothing matches your child’s current size.


  • Parents provide formula/food, Pre-cut Vegetables/Fruits, Snacks

  • Diapers/Pullovers and wipes are provided by parents. Any extra diapers or pull-ups used would be charged at $2 per diaper.


Personal Belongings and Electronics Policy: 

To help maintain a safe, organized, and distraction-free environment, children are not permitted to bring expensive electronic devices, valuable toys, or other high-value personal items into the childcare program. This includes, but is not limited to, tablets, gaming devices, smart watches, electronic toys, collectible items, and other costly belongings.

If a child arrives with such items, parents may be asked to take them home. Any items brought into the childcare setting are done so at the family's own risk. The Provider is not responsible for the loss, theft, damage, or malfunction of any personal belongings brought into the home.

By signing this agreement, parents acknowledge and agree to comply with this policy.


Payment Terms for Services Provided:

Hourly Rate - $15 per hour per child (If Less than 20 hours a week)

*Siblings Discount available ($5 additional per child)


Weekly Rate - Please discuss for specific schedule

(Above rate is inclusive of meals and milk for the duration of 6 months - unless otherwise stated) 


Parents can provide additional snacks if they like. 


**Payments are required weekly before the Final Day.

**Pre-Payment Discount is available. Please Inquire.

**Late Payments will be charged $20 per day.

**In the event that you pay via check, and it gets returned, you will be asked to make all future payments in cash only, and a service fee of $45 will be assessed for the returned check.

**If the care is based on weekly rate, the same will be charged irrespective of the days absent.

Dietary Restrictions & Allergy Acknowledgment

Parents/guardians are responsible for providing complete and accurate information regarding their child's food allergies, dietary restrictions, food sensitivities, medical conditions, and any other health concerns that may affect the child's participation in childcare activities.

While reasonable efforts will be made to accommodate disclosed dietary restrictions and allergies and to avoid exposure to known allergens, we cannot guarantee an allergen-free environment. Children may come into contact with foods or substances brought into the childcare setting by other children, visitors, or through normal daily activities.


By acknowledging below, the parent/guardian agrees and accepts the inherent risks associated with food allergies and dietary restrictions. The parent/guardian agrees that Ainza Huda, and anyone in her household shall not be held liable for allergic reactions, adverse food-related incidents, illnesses or other medical issues resulting from exposure to allergens or restricted foods, except in cases of gross negligence or willful misconduct.


The parent/guardian further agrees to provide any necessary emergency medications (such as an epinephrine auto-injector) along with written instructions for their use and to keep all medical and allergy information current and up to date.


I have read, understand, and agree to the above Dietary Restrictions and Allergy Acknowledgment.

In the event of a medical emergency, the Provider may seek emergency medical treatment for the child if the Parent/Guardian cannot be reached immediately. Any medical expenses incurred shall be the responsibility of the Parent/Guardian.


Please bring a copy of your State-Issued ID at the time of initial drop-off.
